【摘要】 目的：探讨慢性根尖周炎患者进行一次性根管治疗术的急性发作原因和预防措施。方法：慢性根尖周炎共182例，男性84例，女性98例，年龄12岁～75岁。所有病例均应用TRIAUTO-ZX全自动根管治疗仪进行一次性根管治疗。结果：急性炎症的发作率为17.03%。女性急性发作率为17.34%，男性为16.67%(P>0.05)。12岁～34岁急性发作率为20.31%，35岁～54岁为17.95%，55岁～75岁为17.64%，(P<0.05)。前牙急性发作率为14.71%，前磨牙为15.56%，磨牙为17.43%(P>0.05)。器械穿出根尖孔者，急性发作率45.45%，未穿出根尖者为13.12%(P<0.05)。超填者的急性发作率为28.57%，未超填的急性发作率为16.57%(P>0.05)。结论：患者个体内在因素、患牙的解剖病理因素以及术者的治疗和操作是慢性根尖周炎进行根管治疗并发急性炎症的主要因素，因此，避免急性炎症的发生，术者必须熟悉患牙的解剖形态及其变异，严格按照无菌操作技术预备根管，减少根尖周组织的理化及生理性刺激。
